Description

PINELLIAE RHIZOMA (半夏, Banxia) Pinellia Tuber The drug is collected in summer and autumn, washed clean, removed from the outer skin and fibrous roots, and dried in the sun. Description Subspheroidal, sometimes slightly oblique, 0.7–1.6 cm in diameter. Externally white or pale yellow. The apex bears a depressed stem scar, densely surrounded by pitted root scars. The base is obtuse, rounded, and relatively smooth. Texture hard. Fracture white and starchy. Odour slight. Taste pungent, producing a numbing and irritating sensation. Identification (1) Powder Almost white. Starch granules abundant. Simple granules subrounded, semicircular or rounded-polygonal, 2–20 μm in diameter. Hilum slit-shaped, V-shaped, or stellate. Compound granules composed of 2–6 components. Raphides of calcium oxalate embedded in elliptical mucilage cells or scattered freely. Needle crystals 20–144 μm long. Spiral vessels 10–24 μm in diameter. (2) To 1 g of the powder add 10 mL of methanol. Heat under reflux for 30 minutes. Filter. Concentrate the filtrate to about 0.5 mL as the test solution. Dissolve Arginine CRS, Alanine CRS, Valine CRS, and Leucine CRS in 70% methanol to prepare a mixed solution containing 1 mg of each per mL as the reference solution. Carry out the method for Thin-Layer Chromatography (0502), using silica gel G as the coating substance and a mixture of n-butanol, glacial acetic acid, and water (8:3:1) as the mobile phase. Apply separately 5 μL of the test solution and 1 μL of the reference solution to the plate. After development, remove the plate, dry in air, spray with Ninhydrin TS, and heat at 105°C until the bands become distinct. The bands in the chromatogram obtained with the test solution correspond in position and colour to those obtained with the reference solution. (3) To 1 g of the powder add 10 mL of ethanol. Heat under reflux for 1 hour. Filter. Concentrate the filtrate to 0.5 mL as the test solution. Prepare a solution of 1 g of Pinelliae Rhizoma Reference Drug in the same manner as the reference drug solution. Carry out the method for Thin-Layer Chromatography (0502) using silica gel G as the coating substance and a mixture of petroleum ether (60–90°C), ethyl acetate, acetone, and formic acid (30:6:4:0.5) as the mobile phase. Apply separately 5 μL of each solution to the plate. After development, remove the plate and dry in air. Spray with 10% sulfuric acid in ethanol. Heat at 105°C until the bands become distinct. The bands in the chromatogram obtained with the test solution correspond in position and colour to those obtained with the reference drug solution. Water Not more than 13.0% (0832, Method 2). Total Ash Not more than 4.0% (2302). Extractives Carry out the method for determination of water-soluble extractives (2201, Cold Maceration Method). Not less than 7.5%. Prepared Slices Processing Pinelliae Rhizoma (Unprocessed) Break into pieces before use. Description As required for the crude drug. Identification As required for the crude drug. Water, Total Ash, and Extractives As required for the crude drug. Property and Flavor Warm; pungent; toxic. Meridian Tropism Spleen, Stomach, and Lung meridians. Actions To dry dampness, resolve phlegm, redirect rebellious qi downward, stop vomiting, disperse focal distention, and dissipate nodules. Indications Damp-phlegm, cold-phlegm, cough, wheezing, profuse sputum, dizziness and palpitations due to retention of phlegm-fluid, dizziness due to wind-phlegm, phlegm syncope, headache, vomiting, regurgitation, fullness and oppression of the chest and epigastrium, and plum-pit qi. For external use: abscesses, swellings, and phlegm nodules. Administration and Dosage Generally processed products should be used for oral administration. 3–9 g. For external use, an appropriate amount may be ground into juice or powder, mixed with liquor, and applied to the affected area. Precautions and Warnings Incompatible with: Aconiti Radix Aconiti Radix Cocta Aconiti Kusnezoffii Radix Aconiti Kusnezoffii Radix Cocta Aconiti Radix Lateralis Praeparata The unprocessed drug should be used with caution for oral administration. Storage Preserve in a well-ventilated and dry place, protected from insects.

[性味归经]:辛,温;生品有毒。归脾、胃、肺、胆经。 用法用量:煎服,3-9克;外用适量,磨汁涂或研末以酒调敷患处。内服用制半夏。因其性温燥,对阴亏燥咳、血证、热痰等证,当慎用。 [功效主治]:1,化痰止咳平喘,温化寒痰,燥湿,用于脾不化湿,痰涎壅滞所致的痰多,咳嗽气逆,痰饮眩悸,风痰眩晕,痰厥头痛。2,降逆止呕,用于胃气上逆,恶心呕吐。3,消痞散结,用于梅核气,瘿瘤痰核,痈疽肿毒等。亦可用于宫颈癌,癌前病变,食管癌,胃癌,鼻咽癌,皮肤癌。 [现代研究]: 本品含挥发油、脂肪、淀粉、烟碱、粘液质、天门冬氨酸、谷氨酸、谷甾醇、毒芹碱、原白头翁素等。有增进唾液固形成份,缓和咽喉痛,镇咳,抑制矽进展,止呕吐,短暂降压,解毒等作用。生用外治痈肿痰核。 配伍禁忌研究:半夏与乌头属“十八反”之一,二药配伍能使毒性增强。半夏或乌头的单煎液给小鼠灌胃,其死亡率为10.25%和2.635;而二药合煎液或二药的单煎混合液的死亡率为25.45%和20%,其毒性明显增强。但此剂量为临床成人用量的1000倍,而一般用量时,二药合用则未能增强实验动物的毒性反应,故常规用量是较安全的。 妊娠忌用半夏,古文献多有记载。近年实验研究亦证明半夏能终止小鼠早期妊娠。给怀孕7天的小鼠皮下注射半夏蛋白(30mg/kg)可起全数流产。但口服半夏浸膏对家兔在位子宫和子宫瘘管均无明显作用,因口服能被胃蛋白酶降解而失活。临床用半夏治疗妊娠呕吐,亦不乏其例,且疗效肯定,并未见明显毒副反应。 关于孕妇是否禁用半夏问题,近年研究尚未取得一致意见。尤其是在“临床-人”和“实验-动物”的研究上,分歧较大。究其原因,一是动物实验用药量较大,往往超出临床用量的上百倍乃至上千倍,存在剂量-反应关系;二是种属差异;三是实验方法还需进一步深化,除了常规毒理学方法外,还应运用现代毒理学研究方法有针对性地开展特殊毒理和生殖毒理的研究。 [文献]:药品名称:半夏;别名:和姑,地雷公,半眼半夏,地珠半夏,麻芋果,三叶半夏,三叶老,三步跳,麻玉果,燕子尾,制半夏,半夏曲,姜半夏,清半夏,齐州半夏,生半夏,醋炒半夏,戈制半夏,制夏,煮半夏, 《本经》:“主伤寒寒热,心下坚,下气,喉咽肿痛,头眩,胞胀,咳逆,肠鸣,止汗。” 《别录》:“消心腹胸膈痰热满积,咳嗽上气,心下急痛,坚痞,对气呕逆,消痈肿。” 《药性论》:“消痰,下肺气,开胃健脾,止
